Please arrive at least 10 minutes prior to your scheduled time to complete any necessary paperwork.

Our main office can be found at Horatio Professional Center, 541 E. Horatio Avenue, Suite 100, Maitland, FL 32751. 

We also have a satellite office located at Baldwin East Plaza, 1300 N. Semoran Blvd, Suite 110, Orlando, FL. 32807.

You will be clearly notified which office to attend for your appointment.
